面向患者的公立醫院互聯網系統服務設計研究

本研究利用 KJ-AHP 方法和參與式觀察法對公立醫院服務系統進 行了評價,探討了互聯網醫療系統中服務品質與患者滿意度之間的關 係。研究通過文獻收集和分析,對互聯網醫療系統、KJ-AHP 方法、 參與式觀察和非參與式觀察法進行了界定。在國內兩個城市的三家醫 院進行了觀察,其中一家醫院以參與式觀察的方式進行了調研,並對 患者和其他利益相關者進行了半結構化訪談。同時,邀請了四位專家 進行訪談,使用 KJ 法對訪談資訊進行歸納整理,得出面向非住院患 者的公立醫院服務系統評價功能指標,並進行分組,邀請專家進行評 分,使用 AHP 法進行層次分析,得出功能指標的權重排序。隨後, 開展了問卷調查,收集受測者對公立醫院互聯網系統功能指標的評分, 並收集數據。研究結果表明,對互聯網就醫用戶影響最大的指標是醫 療費用成本、時間成本、掛號系統和繳費系統的完善程度以及隱私保 護等。此外,病案列印系統、科室分級管理系統和設備管理系統等系 統也需要進一步提升。最終,根據 AHP 法得到的公立醫院一級功能 指標權重由高到低為手術服務能力、服務安全、檢查服務能力、服務 效率、人員服務能力、環境設施、系統服務能力。本研究的貢獻在於 為互聯網醫療系統的發展提供了理論依據,對互聯網醫療領域建立全 域聯網及產業化有一定的實踐參考意義。此外,研究還為公立醫院提 供了改進服務品質的方向,企業可注重提高醫療費用成本、時間成本、 掛號系統和繳費系統的完善程度以及隱私保護等指標的品質和完善 程度。
This study evaluates the public hospital service system using the KJ- AHP method and participatory observation method and explores the relationship between service quality and patient satisfaction in the internet medical system. Through literature collection and analysis, the study defines the internet medical system, KJ-AHP method, participatory and non-participatory observation methods. Observations were made at three hospitals in two cities in China. One of the hospitals conducted research in a participatory observation manner and semi-structured interviews with patients and other stakeholders. Four experts were invited to conduct interviews and use the KJ method to summarize the interview information, obtain the evaluation function index of the public hospital service system for non-inpatient patients, group them, invite experts to score, use the AHP method for hierarchical analysis, and obtain the weight ranking of the functional indicators. Subsequently, a questionnaire survey was conducted to collect the scores of the functional indicators of the public hospital internet system by the respondents, and data was collected. The results of the study show that the indicators that have the greatest impact on internet medical users are the cost of medical expenses, time costs, the degree of improvement of the registration system and payment system, and privacy protection, among others. In addition, the system for printing medical records, departmental classification management system, and equipment management system also need to be further improved. Finally, the primary functional indicator weights of public hospitals obtained through the AHP method are ranked from high to low. This study contributes to the development of the internet medical system by providing theoretical basis and practical references for the establishment of a globally connected and industrialized internet medical system. In addition, the study provides directions for improving the service quality of public hospitals, and enterprises can focus on improving the quality and completeness of indicators such as the cost of medical expenses, time costs, the degree of improvement of the registration system and payment system, and privacy protection, among others.
